What Is the Quadratus Lumborum (QL)?

The QL is a deep, stabilizing muscle located on both sides of the lower spine. It attaches from the iliac crest (top of the hip) to the 12th rib and the lumbar vertebrae.

Because of its location, the QL is responsible for:

● Supporting and stabilizing the lower spine

● Side-bending the torso

● Lifting one hip during walking

● Supporting breathing by anchoring the rib cage

Even though the QL is small, it has a big job — it supports your body through almost every movement you make.

Why the QL Becomes Tight? The QL is a stability muscle, which means it works constantly to protect your spine. Once it becomes tight, it rarely relaxes on its own.

Here are the most common reasons why:

  • Sitting too much – Long hours of sitting shorten the QL, weaken the core and force the lower back to work overtime. This leads to stiffness, compression and chronic tightness
  • Weak of inactive core muscle – When the deep core (psoas, transverse abdominis and diaphragm) isn’t activated properly, the QL steps in to stabilize the spine. This causes the QL to become over worked, tight and painful.
  • Hip Hiking / Uneven Weight-Bearing – Standing on one leg more than the other, carrying a heavy bag on one side or walking uneven gait patterns can cause pelvis to tilt. The result, one QL gets shorter, tighter and more painful than the other.
  • Shallow or Restricted Breathing – Because the QL attaches to the 12th rib, poor breathing pattersn create tension in the muscle. Stress anxiety and chest breathing make the QL work harder, causing rib tension, lower back stiffness and difficulty taking deep breaths.
  • Emotional Stress & Guarding – The lower back is one of the main body’s “fight or flight” zones. When you’re stressed, the QL contracts to protect your spine. Overtime, this protective pattern becomes deep tension, that feels impossible to release.
  • Lifting, Twisting and Overuse – Improper lifting and repeated twisting can strain the QL. This is especially common in people who work physically demanding jobs, exercise with poor form and those who does repetitive task daily. Leading to trigger points, spasm, and persistent pain.

Symptoms of tight QL:

A tight QL can cause pain that spreads across the entire lower back and pelvis. Clients often report :

  • Sharp or aching pain on one side of the lower back
  • Pain radiating into the hip or glutes
  • Difficulty standing straight
  • Pain when walking or sitting
  • Tightness that feels deep inside
  • Pain that mimics sciatica
